YOUTH PARTICIPATION CONSENT Acknowledgment of Participant: I understand that I am responsible to act in a safe and responsible fashion, to follow the instructions or directions of the persons in charge of the Youth Camp, and to obey requests to comply with safety regulations as directed by the persons in charge of the Youth Camp, including designated leaders. I acknowledge that it is my responsibility wear appropriate safety equipment as directed by the leaders and mandated by law. I agree to behave in a manner that is respectful and appropriate at all times. Also, we are abstaining from any kind of phone usage while at camp. When you arrive to camp we will keep all phones with us throughout the course of camp. We want to block out all distractions that any youth face throughout the week, so they can fully focus on their relationship with Jesus. We believe this will truly help spiritually grow all youth who attend camp. Counselors will have parents phone numbers in case of emergency. By signing this waiver I understand and agree that Realove Church and Youth Camp Directors aren't responsible for accidents.
